Claude Code excessive limits usage in recent updates. Limits have either been reduced, or Claude Code is incorrectly counting tokens

**Bug Description**

Claude Code has been chewing through usage very aggressively over the past few weeks, with no changes in my workflow, skills or mcp tools but my per-session limits on max are being hit VERY quickly.

I always _very_ carefully monitor my token usage, burn rates, context management etc... and I've never seen limits get hit at even half this rate before.

Feels like a bug or Anthropic has changed something in their billing / token counting that is messing over Max users.
