[BUG] Claude would ask questions about a plan without a way to see the plan first

For issue #2 (...), is my proposed restructuring acceptable? ...

At this point I do not have any good way to see the plan so that I have any idea what is the "proposed restructuring" even is!

What Should Happen?

Most obvious solution IMHO - when the Q&A tool is invoked in planning mode, and plan was modified since last presented to the user (or perhaps since last user interaction), have the tool present the current plan alongside questions.
