[Bug] Session history inconsistency: Different Claude Code instances show different conversation counts for same directory

Bug Description
I have 2 claude sessions open. they are on the same directory. one can retrieve 66 conversations, go back to many days, and one can only see 10 conversations, and not even the 10 most recent. so I suspect the session history is corrupted somehow. while i am typing this. I realize the first session can now retrieve 484 conversations. this one is still 10. and if i open another claude window, it's still 10.
