[FEATURE] new command `/rate` to allow providing a rating at any time.

Problem Statement

I like that Claude Code sometimes asks me for feedback. I always try to give it.
Though, I really want to give feedback at the very moment that something goes wrong :-)
as well, I would like to give immediate feedback at the very moment that something goes right, too.

I'm sure y'all can use this data. Just thought I'd offer up the idea.

Proposed Solution

Create a new command called /rate that allows the user to input a rating. Perhaps even letting them type in justification for the rating.
