[Bug] Rate limit options repeatedly queried after session pause

Bug Description
Spamming /rate-limit-options after pausing for session rate limits

Environment Info

• Platform: darwin
• Terminal: tmux
• Version: 2.1.3

I had a longer running query that was paused due to session rate limits. I selected "wait until limits reset" and continued the session the following morning. When I returned, claude code had automatically spammed `/rate-limit-options` as a prompt hundreds of times. I didn't notice it right away, as the query finished first, but then my session wasted a bunch of tokens processing `/rate-limit-options` hundreds of times.
