Bug Description
Hook error message not visible since v2.1.20

Body:

• Version: 2.1.20
• Problem: Hook error messages are no longer displayed to the user when a hook blocks an action. Previously the

error message was visible, allowing users to understand why an action was blocked.

• Expected: The hook's error/stderr output should be shown so users know the reason for the block.
• Impact: Users cannot debug or understand why their hooks are rejecting tool calls.
