[BUG] Resume conversation doesnt resume in correct place

What's Wrong?

❯ /feedback sometimes when I exit claude and run claude again with resume of the conversation I just exited it from, then it jumps back into a place where a plan was running, so the context gets flooded from a time in the conversation that might be many prompts ago

I couldnt file it using the /feedback

What Should Happen?

Claude should just open up where it left off, it usually does, but the plan seems to persist in some way and claude thinks its still running the plan.

Exit a named conversation while nothing was running, claude --resume conversation, goes into wrong place in context.
