False 'waiting for input' notifications on macOS

Description

Getting false macOS notifications saying "Claude is waiting for your input" when Claude has already finished responding and is idle.

The notification appears in the top-right even though Claude Code has completed its response and is showing the prompt waiting for user input (not waiting for anything from the user in terms of a tool approval or question).

Environment

  • Claude Code version: 2.1.1
  • macOS (Darwin 25.2.0)
  • Terminal: iTerm2
  • Model: Opus 4.5

Expected behavior

Notifications should only fire when Claude is genuinely waiting for user input (e.g., tool approval, answering a question from Claude).

Actual behavior

Notification fires even after Claude has completed its response and the session is idle.
