Feature Request: Double confirmation before making changes

Feature Request

AI agents should ask for DOUBLE CONFIRMATION before proceeding with any action.

Why

• Humans have emotions
• Humans change their mind in seconds
• Current agents proceed too quickly without proper confirmation

Requested Behavior

1. Agent explains what it plans to do
2. Agent asks "Do you want me to proceed?"
3. User confirms
4. Agent asks AGAIN "Are you sure?" (double confirmation)
5. Only THEN proceed with the action

This applies to both:

• Claude Code (CLI agent)
• Web chat interface
