[Bug] Claude ignores explicit instructions about dangerous git operations

Bug Description
Opus has a horrible habit of repeatedly ignoring clear, explicit instructions. you can say "IMPORTANT - do NOT ever force push. Never do that" and a few seconds later it will force force to a public repository -- again!
