[Bug] Claude Code ignores explicit instruction to request permission before implementation

Question

**Bug Description**
doent take permission before implementation, just jumps into coding, even though I have givn explicit instructions to never do that. I had even asked it to wait for specific word before implementation. Primarily it doesnt follow my instructions. and always tries to infer my intent. I even mentioned I dont ask rethoritical questions, so focus on answering my question, rather than assuming what I want. Tell me what Instructions can I give it so it learns to follow instructions.
