[Bug] Claude ignores worktree requirement in plan despite explicit prompt instruction

Bug Description
Claude's plan doesn't mention the use of a worktreee even though the prompt specifically asked for one. I have to remind claude to use the worktree after claude produces the plan. Without the worktree specifically called out in the plan, the clear and implement option will start working on the current folder and current branch, and this interferes with other work being done on this branch. Not to mention the changes being addressed are the PR changes requested by claude, which are in the worktree branch, not this folder.
