[BUG] Claude Code fails to edit after plan

What's Wrong?

claude code fails to edit after plan is accepted, I am attaching the log so you guys can debug and fix.

What Should Happen?

It should just edit the file as per plan.

look at the attached file because it is large.

Steps to Reproduce

run claude
ask for a change in plan mode
it came with plan then i accepted
and fails
