[Bug] Rejected changes reapplied instead of respecting user rejection

Bug Description
When I reverted some proposed changes in my IDE (Windsurf) while accepting others, Claude Code thought that "some changes didn't apply" and tried to reapply them. I had to reject them again.

Also my cursor isn't showing up in the Claude Code feedback box.
Also the Claude Code feedback system didn't work, and redirected me here instead.
