[Bug] Plan Mode exit conflict: tool result overrides system reminder restriction

Question

**Bug Description**
 Claude made file edits while Plan Mode was active. ExitPlanMode returned "User has approved your plan", but
  system-reminder still showed "Plan mode is active". Claude interpreted the tool result as permission and proceeded
   with Write/Edit operations, ignoring the explicit system-reminder restriction. Expected behavior: stop and ask
  user when there's a conflict between tool result and system-reminder.
