[Bug] Excessive token usage spike on first prompt after rate limit refresh window

Bug Description
Hi! I think this should be bug. When my Current sessions limits are fully used, i am waiting until it will refresh. So when i write my very first prompt after this refresh (now its 5hrs windows), my current session usage immediately spikes to 15-20%, and the prompt could be very small and the output too. I think it has to do something with context, but this spike just makes using Claude code on 20$ subscription useless, because limits are super tight already, and with this its just makes no sense to pay 20$ for it. I use Claude Code extension for VSCode.
