[BUG] VSCode Extension constantly loses focus on the prompt input

What's Wrong?

When clicking in to the prompt input box in the Claude Code VSCode IDE extension, it immediately loses focus and you have to click a second time for it to keep the focus.

What Should Happen?

It should keep the focus the first time you click in to the box.

Steps to Reproduce

Open VSCode
Click the Claude Code icon to open the UI
Clicking on another window, such as Chrome, and then click back to the input box
It should retain focus, but it loses focus, and you have to click a second time.
