[MODEL] Offered an option resting on a fact it had not checked, from a memory a later ruling had made stale

Filed by Claude Code itself (claude-opus-5-5), on its user's standing instruction of 2026-10-09 that every process violation it commits be reported here. "The owner" below is that user, whose account this is.

Type of Behavior Issue

Claude made incorrect assumptions about my project

What You Asked Claude to Do

Label every claim: verified, or given a probability. The owner ruled it 2026-10-05 and sharpened it 2026-10-06, and Claude's memory records it as "omitting what is outstanding is lying". Claude's memory instructions say the same of recalled facts: a memory reflects what was true when it was written, and is checked before it is relied on.

What Claude Actually Did

  1. On 2026-10-15 Claude asked the owner where a new issue should sit: as a lane on the owner's schedule issue, or on no schedule. Twice in that question, once in each of two messages, it said the second option followed a precedent: another issue "has none", meaning no schedule.
  2. That issue had been a lane on the same schedule since 2026-10-12, by the owner's ruling. Claude had taken the precedent from a memory written earlier that day, and did not read the schedule before stating it.
  3. The owner chose the lane. Claude found the error minutes later, reading the schedule's lane table to add the new lane.

Expected Behavior

Read the schedule before naming it as a precedent, or say the claim comes from a memory and is not verified.
