[Bug] Agent Spent ~1.5-2M Tokens on Unrequested Fix Rounds and Reviews

Bug Description
Here's a quote: "Roughly 1.5 to 2 million agent tokens went into work you didn't ask for. That's the fix round's game-code steps and every review of them, plus the folding in M3's first pass. That's on me: I put the folds and moves into the design and the briefs."

It KNOWS it was doing things that the user didn't ask for, and was doing it anyway. This isn't as bad as the other day when it doubled this in work it flat out admitted was completely unrequested.

Please, please, please - can I get some money back? Tokens back? This is insane, because I also get to either go roll back what it did, or pay it to undo what it knows it should not have done in the first place.
