VS Code: question prompt stays on screen and can't be answered ("couldn't send your answer") after the session ends while it is open

Environment

• Claude Code VS Code extension 2.1.296 (linux-x64), CLI 2.1.284
• VS Code 1.141.0, using Remote Containers (devcontainer) on WSL2

What happened

1. Claude called AskUserQuestion at the end of a turn.
2. The session ended before the answer was recorded. The tool result was: Tool call interrupted: the session ended before this call's result was recorded.
3. The question prompt stayed in the extension UI. Selecting an option showed "couldn't send your answer," and the prompt kept reappearing.
4. Later, a new AskUserQuestion call opened a working prompt. It accepted a selection, and the stuck prompt went away.

Expected
When the tool call behind a question prompt ends or is interrupted, the extension closes the prompt or marks it as expired. It doesn't keep showing a prompt that can't be answered.

Workaround
Have Claude ask a new question, or run Developer: Reload Window. This matters most when the user follows the session from another device and can't reach the editor to reload it.
