[BUG] Claude Code answered itself and wrote code based on its own answer

What's Wrong?

Claude asked a yes/no question, answered it as "Yes, fix it." attributing that answer to itself as if it were the user, then proceeded with a significant code change without actual user approval.

What Should Happen?

CC should not have made code changes without explicit approval

Steps to Reproduce

I don't have concise steps to reproduce. What I did was ask CC to research an issue, it found the issue and proceeded to fix the code after asking for permission, then answering itself to fix the code.
