VS Code extension: effort level sometimes missing from the chat UI

What's Wrong?

In the VS Code extension, the effort indicator sometimes doesn't appear in a chat's input bar. Other chats in the same window show it. It doesn't seem to depend on the model: the same model shows it in one chat and not in another.

What Should Happen?

Every chat shows the effort level.

Steps to Reproduce

1. Open the Claude Code extension in VS Code.
2. Open several chats.
3. Occasionally (rarely), the effort indicator is missing from the input bar next to the model chip. I don't know what triggers it.
