[Bug] Agent orchestration failures causing zombie agents and lack of health checks

Bug Description
The frquency of agent just going zombie, and main agent not be able to tell when or think to sanity check is getting wild. I usually task my main agent with a primary role of orchestration, which its failing at often and repeatadly. Additionally script run by agents seems much more unstable/unreliable.
