[Bug] Claude Code sessions leak tool uses and diffs across VS Code split terminals

Bug Description
I'm in VS code, using integrated terminal, with 2 separate terminal sessions in split view. One claude agent in each, running separate tasks on the same repo. I asked something in one terminal, claude did the thing, but in the other, separate, unrelated terminal, I saw the diff and tool uses in that session, which was unexpected and potential bug.
