[BUG] Authorization tokens list doesn't update after Revoke Access (token only disappears on reload)

Preflight Checklist

  • [x] I have searched existing issues and this hasn't been reported yet
  • [x] This is a single bug report
  • [x] I am using the latest version of Claude Code

What's Wrong?

On the Claude Code settings page (Settings → Claude Code → Authorization tokens), clicking Revoke Access on a token appears to do nothing. There's no confirmation, no error, and the token stays in the list. After reloading the page, the token is gone, so the revoke works but the list never updates.

It happens the same way in the Claude Desktop app and on claude.ai in a web browser. With several tokens to clean up, it looks broken, and you can't tell which ones you've already revoked without reloading after each click.

What Should Happen?

After Revoke Access, the token should disappear from the list right away (or show a confirmation).

Error Messages/Logs

None shown in the UI.

Steps to Reproduce

  1. Open Settings → Claude Code in the Claude Desktop app, or claude.ai/settings/claude-code in a browser.
  2. Under Authorization tokens, click Revoke Access on any token.
  3. Nothing visible happens; the token is still listed.
  4. Reload the page; the token is gone.

Is this a regression?
