Cannot revoke Claude Code authorization tokens

What's Wrong?
Unable to revoke Claude Code authorization tokens from Settings → Claude Code. Clicking the trash icon and confirming "Revoke access" in the dialog does nothing — the token remains in the list. I have 15 active tokens across 3 pages and cannot remove any of them.
What Should Happen?
Expected Behavior
The token should be revoked and removed from the list.
Actual Behavior
Nothing happens. The token stays in the list after confirming revocation.

- Go to claude.ai → Settings → Claude Code
- Click the trash icon on any authorization token
- Confirm "Revoke access" in the confirmation dialog
- Token remains — not revoked

Troubleshooting Attempted
- Tried multiple browsers (Chrome, Firefox)
- Hard refresh (Ctrl+Shift+R) after revoking
- Cleared browser cache and re-logged in
- Changed account password
- None of these resolved the issue
Security Concern
I suspect unauthorized access to my account and I'm unable to revoke these tokens to secure it. I also cannot reach Anthropic support — the support chat widget redirects to claude.ai instead of letting me submit a ticket while logged in.

- 15 active Claude Code tokens (across 3 pages)
