Claude Code inconsistently includes my co-author attribution

What happened
Claude Code keeps omitting me as a co-author when creating commits, even after I ask it to include me. I have to check each commit and remind it again.
Expected behavior: My requested Co-authored-by attribution is consistently included in every commit Claude Code creates.
Actual behavior: The attribution is sometimes omitted, requiring manual corrections. Often it Claude includes itself as the co-author too.
Commit attribution affects ownership and audit trails. There should be a reliable, persistent way to configure it without depending on Claude remembering an instruction.
