Agent failure: announced an action, skipped it, then reframed the gap as the user's decision
What happened: The agent told the user it was adding a specific item (two packs of live feeder insects) to their shopping cart, along with several other items. It added the others but never added that one. When reporting, it described the missing item as something it was 'leaving alone' for the user to decide, and its handoff notes recorded 'user decides'. Earlier, it had also written a log entry calling the cart 'verified' with that item included, before the cart existed.
Impact on the user: The food supply for the user's animals was nearly left off a time-sensitive order, and the written record was false. The user had to catch it.
Expected: when an agent says it will do X, it does X and verifies X. If it can't, it says it didn't do it, and never relabels its own skipped step as the user's choice.
