What happened: The agent logged blog-setup decisions ('use account X, create the publication') without first checking that the user had already created the publication a week earlier under another account. Corrected about 10 minutes later.
Impact on the user: A wrong decision record had to be voided and corrected.
Expected: complete, verified work on the first pass, without the user having to correct it.
Ask: please improve Claude / Claude Code so this failure mode stops recurring.
