Bug Description
The idea that I take the time to create skills, and the primary purpose they serve is for claude to point to and say, "You're right, I ignored what I was supposed to do", is absolutely ridiculous, and is making Claude Code impossible to use productively for entire days on end.

Why even have "skills" if they don't load in the exact situations they're supposed to? What good is a coding assistant that always tells me, "I didn't do what I'm supposed to, and there's no way for you to get me to. Now, what would you like to do?"

And these same lapses happen not just with skills, but, even when things are in CLAUDE.md "You're absolutely right, in fact we have a rule against that in CLAUDE.md, and I loaded it during this session. I just ignored it." And, yes, I've tried both going into excruciating detail in my rules, I've tried slimming the files down so they're as lean as possible, I've tried progressive loading schemes. I've tried /doctor. I keep sessions fairly short and start fresh when I can.

"SKILLS" AND "CLAUDE.MD" DON'T WORK, AND THEY NEED TO.
