[BUG] [A11y] When using manual mode with the screen reader mode (using NVDA, but it shouldn't matter), the diff is not shown when Claude edits files

Preflight Checklist

  • [x] I have searched existing issues and this hasn't been reported yet
  • [x] This is a single bug report (please file separate reports for different bugs)
  • [x] I am using the latest version of Claude Code

What's Wrong?

When Claude edits existing files, if manual mode is on and screen reader mode is on, the diff is never shown, only the dialog to accept or reject the file edit.

What Should Happen?

The diff should be shown before the dialog, just like in the non-screen reader mode.

Steps to Reproduce

  1. Turn on screen reader mode, with --ax-screen-reader, or via settings, setting that flag to true.
  2. Switch to manual mode.
  3. Ask Claude to write a hello world. The diff is there before accepting.
  4. Ask it to ask the user's name and print it. The diff is not there before accepting.
