[Bug] iOS push notifications not delivered from Claude Code except Remote Control connection

Bug Description
Mobile push notifications from Claude Code never arrive on iOS, except the "Remote Control connected" one.

Claude Code 2.1.285 on macOS, Remote Control connected. Around 20:00 on 2026-09-30 the PushNotification tool returned "Terminal notification sent. Mobile push requested." Nothing reached my iPhone. Permission prompt pushes don't arrive either, even after leaving the terminal idle for 20+ minutes.

Already checked:

1. Push when actions required and Push when Claude decides are both true in /config. I toggled both off and on to sync them to the server.
2. /config shows no "No mobile registered" warning.
3. The tengu_kairos_push_notifications and tengu_kairos_input_needed_push flags are both true.
4. In the Claude iOS app, every notification switch is on, including Code and project updates and Code permission requests.
5. Focus mode isn't the cause. The "Remote Control connected" notification arrives

So the phone can receive Claude pushes, and the notification reaches the server through Remote Control, but it's never delivered.
