Desktop (macOS): sessions-bridge dies at the 12-hour ingress-token refresh (code 4090), and Remote Control sessions stay offline until the app restarts
Environment: Claude desktop app 2.16120.0 (Claude Code 2.1.284), macOS 26.5.1, Apple Silicon. An always-on Mac mini hosting Code sessions with Remote Control on by default, reached from a second Mac and a phone on the same account.
What happens: once or twice a day, every Remote Control session hosted on this Mac goes offline on other devices at the same moment, and they disappear from the other Mac's sidebar. The sessions keep running locally. They only reappear after the desktop app restarts, anywhere from 9 to 45 hours later. The host never sleeps, so the "system resume" recovery path never fires.
Logs (~/Library/Logs/Claude/main.log), every occurrence has the same shape:
[sessions-bridge] Forcing session_ingress_token refresh for cse_<redacted> (trigger=proactive_timer)
[sessions-api] Reconnecting session cse_<redacted> to environment env_<redacted>
[sessions-bridge] Session cse_<redacted> already active; refreshing session_ingress_token from duplicate work cse_<redacted>
[sessions-bridge] Transport permanently closed for session cse_<redacted> code=4090
[sessions-bridge] Transport reconnect cap reached for session cse_<redacted> (6 attempts); triggering backend redispatch
[error] [sessions-bridge] Cap-redispatch budget exhausted for cse_<redacted> (2); transport stays dead until app restart or system resume
Frequency: 15 occurrences from 2026-09-20 to 2026-09-30, and daily since at least 2026-08-30. There were no network errors in the minutes before any of them. It happens roughly 12 hours after the last successful reconnect, which matches the refresh timer (43200s).
Expected: the refresh treats "already active" as success, or the bridge keeps retrying with backoff instead of staying dead until a restart.
Related: a new session whose first-turn Remote Control enable fails (for example during a brief claude.ai auth error) retries only when the next message is sent in that session. There is no background retry.
