Bug Description
I am having repeated issues in multiple sessions due to the permissions auto-mode classifier returning an empty verdict. It happened in two separate sessions on consecutive days, on two platforms. This is especially problematic in headless -p runs. A transient classifier error stops a headless -p run with no retry or fallback, so unattended jobs die.

Both sessions were on Claude Code 2.1.28x, one in WSL and one on native Windows, which suggests a
problem on the service side rather than my setup.

Yesterday's -p session (WSL Ubuntu 24.04)

  • Session ID: 0f07d316-cee2-49ed-b934-0eaa51b90e6e. I restarted with manual permissions and the run continued in this same session.
  • Setup: version 2.1.284, headless claude -p run, permission mode auto
  • Errors: 13 classifier failures, all times UTC on 2026-09-29:
  • First burst: 6 failures between 18:05:15 and 18:06:11
  • Second burst: 7 failures between 18:17:49 and 18:19:25
  • Ending: the session stopped at 18:19:36, 11 seconds after the last failure.
  • Restart: it resumed at 18:59:28 in default (manual) mode and finished without further trouble.

Today's session (native Windows)

  • Session ID: fd6998b7-e9ba-4dac-8dd3-14e5418c262b
  • Setup: version 2.1.285, interactive, permission mode auto
  • Errors: 5 classifier failures on Bash calls while running the /doctor diagnostic. File reads kept working the whole time.
