[BUG] DesignSync upload prompts for confirmation every time despite auto mode and explicit allow rules

What happens

In auto mode, every DesignSync upload to a claude.ai/design design-system project shows a confirmation prompt, repeatedly within the same session:

Design: Upload design system (4 to upload)(...)
Review what will be uploaded before continuing.

Do you want to proceed?
> 1. Yes
  2. No

This blocks autonomous runs of the /design-sync flow: each component upload waits for a manual "Yes".

Expected

No prompt, because the tool is explicitly allowed and auto mode is on. Or at least a "Yes, don't ask again for this session/project" option.

Settings (~/.claude/settings.json)

{
  "permissions": {
    "allow": ["DesignSync"],
    "defaultMode": "auto"
  },
  "skipAutoPermissionPrompt": true,
  "autoMode": {
    "allow": ["$defaults", "DesignSync"]
  }
}

The allow rules for DesignSync in both permissions.allow and autoMode.allow have no effect on this prompt.

Repro

  1. Use the settings above.
  2. Start a session in auto mode and run the /design-sync skill against a design-system project on claude.ai/design.
  3. Every upload call prompts for confirmation.

Environment

  • Claude Code 2.1.285
  • macOS (Darwin 25.6.0)
