[Bug] Subagents hang waiting for approval on external CLAUDE.md imports

Bug Description
Sometime subagents are stuck indefinitely because claude asks to import external CLAUDE.md (I don't remember exactly the words). I spot them connecting to the tmux swarm and unlocking them one by one clicking on "approve"
