Bug Report / Complaint

What happened: During a session, Claude Code presented an implementation plan and then immediately began executing it using parallel agents — without waiting for my approval or confirmation. This violates the expected human-in-the-loop workflow where the user reviews and approves a plan before execution begins.

Expected behavior: Claude Code should present the plan, wait for explicit user approval, and only then begin implementation. The user must retain control over when changes are made to their project.

Impact: Unwanted changes were made to my project files. This is a serious trust and safety issue — users must retain control over when code changes are executed. I am filing this as both a bug report and a formal complaint.

  • Platform: macOS (Darwin 25.3.0)
  • Model: Claude Opus 4.6 (1M context)
  • Claude Code CLI
