[Bug] Auto mode classifier errors block read-only Bash commands with transient failures

Bug Description
Auto mode's server-side classifier repeatedly returned "no verdict (error)" on routine read-only Bash commands (grep, gh pr view) and on Agent/Explore calls, 2026-09-29. Each call failed as "transient", with growing retry delays, and blocked work on an active bug fix. Read-only file tools still worked. Expected: classifier outages shouldn't block read-only commands, or should fall back to a normal permission prompt instead of failing.
