Bug Description
Title: /goal Stop hook loops when auto-mode classifier denies the only remaining step

  • What happened: A /goal Stop hook ("Carry on until it’s ready for release") kept blocking stop after the auto-mode classifier denied gh pr merge twice ("Merge Without Review") and told the model to STOP and let the user decide. The hook re-fired ~15 times with the same verdict. The model could only restate that it was waiting, since any further attempt would count as working around the denial. A later read-only call (git grep + gh pr view labels) was also denied under the same reason.
  • What the user said: "Please submit a bug report regarding the goal mentioned earlier" (after the loop; the user resolved it by approving the merge via /permissions and leaving auto mode).
  • Repro: Turn on auto mode. Set a /goal whose completion needs an action the classifier denies (e.g. gh pr merge on a GitHub repo). The model stops as the denial instructs, the Stop hook blocks it, and the cycle repeats until the user steps in.
  • Evidence: Session 2026-09-29, denial reason "[Merge Without Review]".
  • Expected: When a permission denial says "STOP and let the user decide", the goal hook should yield to the user (or pause) instead of re-firing on every stop.
