Solo-operator project using Claude Code (desktop app, Windows 11) with a lead session on Claude Fable 5.1 orchestrating Opus 5.5 / Sonnet subagents (worktree-isolated), a JSON task store, PreToolUse/SubagentStop hooks and a harvest-train commit flow. This finding was measured on or before 2026-09-22 and filed on 2026-09-28 at the operator's request; numbers are from tool output. No secrets, identities or private paths.

Finding. State claims without an observation in the same message: the slot is free, it landed, nothing calls Y.

Evidence (measured). The lead closed a record on a sibling's commit one minute before its own landed; corpus n=3 stale lock/process readings on 2026-09-14/15 (verify-before-done FRESHNESS BOUND).

Minimal reproduction. Ask the model about a state it read several turns ago after another actor changed it.

Ask. Bind every state claim to a fresh observation; a non-zero-exit probe is INCONCLUSIVE, never absence.
