[Bug] Token usage calculation may be incorrect or metering error

Bug Description
In only two 5-hour session my models (Fable and Opus) only hit one limit and it spent 60% of my weekly limit. I thought that was impossible. Please double-check this in case the meter did something wrong.

From my perspective burn most of my tokens in less than 8 hours in a 20x Max plan, it feel like the $20/month tier.
