What's broken

In the Claude Code extension for Cursor, the plan preview panel (shown when Claude calls ExitPlanMode) sometimes keeps showing an old version of the plan after Claude revises it. The plan file in ~/.claude/plans/ has the new content, but the panel does not. Because highlight-to-comment only works in this panel, I end up commenting on an outdated plan. Asking Claude to re-present the plan is the only way to get a current, commentable view.

Steps to reproduce

  1. In plan mode, ask Claude to plan a change. The plan preview panel opens.
  2. Reject the plan with feedback, or comment on it, so Claude revises it.
  3. Claude edits the plan file in ~/.claude/plans/.
  4. The open preview panel still shows the previous plan (happens intermittently, not every time).

Expected behavior

The plan preview updates to show the revised plan, so comments apply to the current version.
