[FEATURE] VS Code extension: render Markdown formatting in the prompt input while typing

Problem Statement
The Claude Code VS Code extension shows raw Markdown syntax while typing in the prompt box.
For example, typing - leaves the literal characters visible instead of turning the line into a rendered bullet-list item. Claude Desktop and Claude Web already provide this kind of live Markdown formatting, so the difference is a small but noticeable UX inconsistency when switching between clients.
Proposed Solution
Support live Markdown rendering in the VS Code prompt input as well, ideally matching the behavior of Claude Desktop/Web for common formatting such as:
- bulleted lists
- numbered lists
- other basic Markdown formatting where appropriate
This is specifically about formatting while composing the prompt, not Markdown rendering after the message has been submitted.
