Permission dialog: button order shifts, causing accidental Deny/Approve-All clicks
Description
The permission prompt's button layout is not stable across prompts. Sometimes an "Approve All" (or similar) option is inserted at the position where "Deny" previously appeared, so a user clicking in the same spot they normally would (muscle memory) ends up clicking a different action than intended.
Impact
I hit this myself: I meant to deny a single tool call, but because the layout had shifted, my click landed on "Approve All" instead. This is a real risk for anyone acting quickly — it can approve something that was meant to be denied.
Suggested fix
Keep button positions fixed and consistent across all permission prompts — in particular, always place "Deny" in the same (e.g. last/rightmost) position regardless of which other options (Approve, Approve All, etc.) are present, so users can rely on consistent muscle memory.
