[BUG] Haiku not available in VS Code Extension

What's Wrong?

When I'm working from the terminal, I can access Haiku 4.5 without any issues, but when I use the VS Code extension, the option doesn't appear in the model selector. I also can't set it by trying to use /model Haiku

What Should Happen?

I should be able to select Haiku 4.5 from the /model command.

Steps to Reproduce

1. Open Claude Code extension in VS Code
2. Try to access the model selector or type /model (or /model haiku)
3. Notice that Haiku 4.5 is not available as an option, even though it works fine when using Claude Code from the terminal
