Bug Report
Description
Since ~Sep 16, 2026, Claude no longer retains file paths and repository locations established earlier in the same conversation. This directly causes users to commit files to wrong repositories.
Steps to Reproduce
- Establish repository paths in conversation (e.g. working on two repos: ps4-suid-scanner and ps4-13xx-research)
- Ask Claude to generate git commands for those repos later in the same conversation
- Claude outputs generic placeholder paths like
~/ruta/repo-nameinstead of actual paths - Ask Claude about files you committed minutes ago in the same session
- Claude claims the files do not exist
Expected Behavior
Claude should remember file paths and repository context within the same conversation, as it did before ~Sep 16, 2026.
Actual Behavior
- Outputs placeholder paths instead of real ones
- Claims files do not exist when user committed them minutes earlier
- User must repeat context that was already established
Impact
User committed 1400_offsets.txt to the wrong repository because Claude confused which repo it belonged to. This is a direct consequence of the memory loss within a single session.
Environment
- Models affected: Opus 4.6, Opus 5.5, Sonnet 4.6
- Platform: claude.ai (mobile + desktop)
- Regression start: ~September 16, 2026
