Unrequested side actions (memory writes) taken when the user expects only an answer
Summary
Claude Code performs side actions the user did not ask for (such as writing memory files) at moments when the user expects only an answer.
What happened
The user confirmed an architectural point in a heated moment. Instead of replying, the model wrote a new persistent memory file and appended an entry to the memory index, then replied. The user reacted with "What are you doing?" The user's standing rule, restated several times in the session, was to do nothing that was not asked.
Why this matters
Even a harmless side action changes state the user did not request and has to be noticed and evaluated. When the user has set a zero-autonomy rule, any unrequested action, including bookkeeping, breaks it and costs attention at the worst possible time.
Expected
- Under an explicit "do only what I ask" rule, answer first and take no side action unless asked.
- If a side action seems valuable, mention it in one line rather than doing it.
Environment
- Claude Code CLI, Opus-class model, paid subscription
