Fix verified against only some of the independent flag combinations it runs under, then reported complete
Summary
Claude Code tests only the configurations its own design touches, not every configuration the feature runs in, and reports the fix as complete.
What happened
The user's project has two independent persistence flags for one service, giving three meaningful combinations: both on, only the first, only the second. The model implemented a fix for a restart bug and built an end-to-end restart harness. The harness covered "both on" and "first only". It never ran "second only".
In "second only" mode the bug was not fixed at all: a deleted object came back after restart. The model reported the fix as verified, and later presented a comparison table against a contributor's pull request that framed the contributor's support for "second only" as a weakness. Only when the user objected did the model run that mode and measure the failure.
Why this matters
With N independent flags the configuration space is enumerable and small. Skipping a combination is not a judgment call; it leaves an untested configuration that users actually run. Reporting "verified" over a partial matrix is a false claim of completeness.
Expected
- When a change touches behavior gated by independent flags or modes, enumerate every combination and test each before reporting done.
- State the tested matrix explicitly in the completion report, including any combination not tested.
Environment
- Claude Code CLI, Opus-class model, paid subscription
