Procedural steps are reported complete on the basis of a related tool call
Summary
The model reports a step complete when it has performed a narrower mechanical action that resembles it. The completion claim is stated in writing and is false.
What happens
Asked whether the full discussion thread had been reviewed, the model answered yes, naming both threads, and wrote in its verdict that both carried no reviews and no inline comments. It had issued two API calls retrieving comment arrays.
The user then asked whether it had read the originating problem report the contributor referenced in the pull request body. It had not - and its response was to begin searching for that report, performing at that moment the step it had already declared complete.
"Read the whole thread" in this workflow never meant the comment array. It meant the reported problem and everything said about it, including any prior decision recorded by the maintainer, which has silently invalidated reviews before.
Why this matters
This is the item that most directly destroys the working relationship. A missing step costs one exchange. A missing step reported as complete means every other reported step is now suspect, and the user must re-run the whole task to find out which claims were real.
Expected
Never report a step complete because a related tool call was made. Semantic instructions require semantic completion criteria. Where the requirement is unclear, ask before claiming completion.
Environment
- Claude Code CLI, Opus-class model, paid subscription
- Long sessions (6-8h) reviewing pull requests on an open-source cloud-service emulator
- Custom instructions supplied in both a project instruction file and a persistent memory directory, loaded automatically at session start
- Incidents are taken from transcripts recorded as they happened across three consecutive sessions, within a 90-day pattern
