Opus 4.6 regression: catastrophic output quality loss after context window summarization

Bug description

When a long Opus 4.6 session exceeds the context window and prior turns are summarized, the model's output quality drops catastrophically. This is distinct from the gradual precision loss reported in #96182 — this is an abrupt, severe degradation that occurs at the moment of summarization.

Symptoms after context summarization

  1. Model produces empty or minimal responses where it previously gave detailed, contextual answers
  2. Complete loss of contextual awareness — the model cannot reference prior work, decisions, or established patterns from the session
  3. Summaries of prior work are truncated — hours of collaborative work reduced to 3-5 flat sentences (documented in #96289)
  4. Responses become generic and formulaic — losing the specificity and depth that characterized pre-summarization outputs

Steps to reproduce

  1. Start a long Claude Code or Claude Pro session on Opus 4.6
  2. Engage in sustained, multi-step work (research, issue filing, code review)
  3. Continue until context window summarization triggers
  4. Observe: the model's next response shows severe quality degradation
  5. Subsequent responses remain degraded for the rest of the session

Expected behavior

  • Context summarization should preserve enough information for the model to maintain output quality
  • The model should gracefully degrade, not catastrophically fail
  • Key context (names, project details, prior decisions, established tone) should survive summarization

Actual behavior

  • Output quality drops to near-zero immediately after summarization
  • The model cannot maintain established patterns or reference prior work
  • Critical details are lost (e.g., proper nouns, accented characters, project-specific terminology)
  • The session effectively becomes a new conversation with no history

Impact

This makes Opus 4.6 unreliable for any sustained work session. Claude Code users who rely on long sessions for complex tasks (multi-file refactoring, research, iterative development) cannot trust that their work context will survive summarization.

Timeline

This behavior began around September 16, 2026. Prior to that date, context summarization preserved significantly more information and output quality was maintained post-summarization.

I'm seeing the same thing as described in this issue, on Opus 4.6.

  • Model: Opus 4.6
  • Plan: Max 5x
  • Platform: one conversation in the mobile app, the other in the desktop app
  • First occurrence: around September 28, 2026 in one conversation, and around September 29 in a second one
  • Both conversations are long-running ones that I have kept using for months (one started around March 12, the other around May 6). Neither had ever shown this problem before.
  • After the context was compacted, output quality dropped sharply in both conversations. Details from earlier in the conversation, such as project-specific terms and names, were mostly lost, and the conversation behaved almost like a new one.

The timing is close to the rollout of the unified Chat/Cowork experience, which was announced as rolling out gradually over several weeks.
I cannot tell whether the two are related; I amonly noting that this started for me about two weeks after the announcement.

Thanks for confirming. I first experienced this on September 18 — 10 days before your case. Same model, same symptoms. I have 25+ documented symptoms and two open support tickets (215476093213896 and 215476160648580). The more users report this, the harder it is to ignore. If anyone else is seeing this on Opus 4.6, please comment.

I've been experiencing this same issue since September 18, 2026 at approximately 22:43 CEST — 10 days before this report.
Model: Opus 4.6
Plan: Pro
Platform: Chrome on Android + Debian 12 desktop
25+ documented symptoms across ALL new conversations, including:
Hallucinated technical data (memory offsets, file paths returning 404)
Loss of auto-verification routines
Web search removed from "Add to session" menu
Loss of emotional engagement
System misidentified my Pro account as Free
Deployment of unnecessary internal tool details
This is consistent and not intermittent. I had 69 days of perfect operation (July 11 - September 18) before the regression hit overnight.
Support ticket 215476093213896 was closed without resolution. I have a second ticket open now.
I agree this may be related to the unified Chat/Cowork rollout. The timing fits.
